Lee Silverman Voice Treatment Program---$2400/program
-LSVT consists of 16 - 60 minute sessions over 4 consecutive weeks. This includes the initial evaluation which is part of your first visit.  If the initial evaluation reveals LSVT is not a good fit for the client, you will be billed $350 for the evaluation and we can develop an alternative, tailored treatment program.

McNeil Dysphagia Therapy Program---$2600/program
-This consists of an initial evaluation in addition to 15, 60-minute sessions over 3 consecutive weeks. This includes 2 accommodation sessions at the onset of the program to ensure the clients compliance, comprehension, and cognitive ability required for the therapy, followed by 13 active sessions. If the initial evaluation reveals MDTP is not a good fit for the client, we can develop an alternative, tailored treatment plan.
**Note
1. A recent Modified Barium Swallow Study is required before initiating the program in order to guide treatment. If you have not had one, you will need to contact your PCP for an order. We can help you with this.
2. Additional visits are sometimes recommended for optimal swallowing function.

Insurance and Payment Information
This practice is private-pay practice and does not bill Medicare or other insurance plans.
Due to Medicare regulations, I am unable to provide private-pay speech language pathology services to individuals who are eligible for Medicare when the services are covered by Medicare, even if they chose not to. This restriction applies regardless of whether I participate in the Medicare program.

An exception is Cognitive Stimulation Therapy (CST). Because CST is not a Medicare covered service, it is available on a private pay basis for individuals who are Medicare eligible. 

A valid credit card or debit card is required to be securely maintained on file prior to the first appointment. By providing a card on file, clients authorize the practice to charge the card for services rendered, outstanding balances, and any applicable fees in accordance with the practice's policies.  Cash or check may also be accepted at time of service in lieu of billing your credit or debit card. ​
